Misdiagnosed with a variety of illnesses, quick-witted high school student Adam (Charlie Plummer) finally learns why he’s been experiencing visual and auditory hallucinations: He suffers from a mental illness. After a psychotic episode in chemistry class, Adam is expelled in the middle of his senior year, jeopardizing his goal of attending culinary college in the fall. But with the help of an experimental new drug and the support of his loving mother Beth (Molly Parker), Adam enrolls in a nearby Catholic school to finish out the term and earn his diploma.
Although Adam’s delusions cause him to see and hear imaginary characters — including an ethereal and eternally optimistic young beauty (AnnaSophia Robb), a cigar-chomping, bat-wielding tough guy (Lobo Sebastian) and a hilariously horny teenage boy who shows up at the most inopportune moments (Devon Bostick) — he keeps his mental illness secret and does his best to avoid attention. But after striking up a surprising friendship with his brilliant classmate Maya (Taylor Russell), Adam begins to experience a sense of hope and belonging for the first time in years.
Unfortunately, the trial drug soon leads to a number of serious side effects. Desperate to maintain a semblance of normalcy, Adam secretly stops taking his meds, causing his visions to flood back with renewed vigor. Making matters worse, his mom announces that she and her live-in boyfriend Paul (Walton Goggins) are expecting a baby. Already resentful of their relationship, Adam discovers that Paul has been emailing his school’s headmistress (Beth Grant) behind his back.
After he seeks guidance from a kind and perceptive priest at school (Andy Garcia), Adam’s struggle for sanity takes a worrisome turn when a voice in his head suggests a permanent solution to his problems. With prom and graduation fast approaching, and his budding romance with Maya hanging in the balance, Adam must make a difficult choice: run from his fears and surrender to his incurable illness, or admit his flaws and reveal his true self to the people who love him.
